About Home Connections Palliative Care Program and Hospice Buffalo

Home Connections Palliative Care Program offers home-based palliative (comfort) care for patients and families facing serious illness. The services are fully covered by some insurances including Senior Blue, BlueCross BlueShield of WNY, and Independent Health* Private pay is also accepted.

What Makes Home Connections Unique from other Home Care programs?

-Patients do not need to be home-bound or have a skilled need

-In-home palliative care physician visit is available if requested by the patient’s physician

-Trained volunteers are available to help at home

-Expert help with health care proxy/advance care planning

-RN, Social Work visits offered along with 24/7 on call nurse

-Help with medication questions, education about disease process and help in care coordination among health care providers

Home Connections is a program of Caring Hearts Home Care, a licensed home health care agency of The Center for Hospice & Palliative Care, one of the nation’s largest hospice and palliative care organizations. The Center for Hospice & Palliative Care is based in Cheektowaga, NY, and comprises a number of health care agencies, including Hospice Buffalo, Inc.

Hospice Buffalo provides medical care, emotional and spiritual support to any individual with serious illness and their family. Patients who have approximately six months or less to live are eligible for Hospice services.
Hospice is a fully covered Medicare benefit. Most insurance plans cover hospice services.
